1.5 – 1.9 times baseline serum creatinine or

Increase in serum creatinine >= 4 mg/dl (>= 353.6 micromol/L) or
Initiation of kidney replacement therapy or
Decrease in eGFR to < 35 mL/min/1.73 sq. m
